What Documents Will I Need To Provide?

DD-214 Member 4.   In order to qualify for the Washington State/Peninsula College Veterans Tuition Waiver (50%), you must provide a true Member 4 copy of your DD-214 and meet eligibility requirements.

Veterans Benefits Confirmation Request Form (Blue Form):   Each Quarter, Please fill out and return to Veterans Services Office requesting certification of enrollment to the VA.  Without this form, your enrollment will not be provided to the VA, and Educational Benefits will not be received.

Transcripts:   Please submit an official copy of your military transcript as well as any transcripts from previous colleges you may have attended.  Forms are available in the Veterans Services Office for your convenience.

Award Letter:  Once you have applied for your VA Educational Benefits and received an Award Letter from the VA, please provide a copy to the Veterans Services Office as soon as possible.

Other documentation you may need:

Transfer of Benefits:  If you have used your GI Bill benefits at an other college or university, in order to transfer those benefits to Peninsula College, you will need to fill out a form 22-1995 or 22-5495 and submit to the Veterans Services Office.

Change of Program:  When changing your program of study, a 22-1995 or 22-5495 form must be on file with the Veterans Services Office.

For Non-resident students: Students who have not established Washington State residency and resided in the state for a period of 12 months or more:
Washington State Drivers License (to prove Washington State domiciliary).

For Dependents: Proof of Washington State domiciliary is needed for Peninsula College to apply the Washington State waiver “Children and Spouse of totally Disabled or POW/MIA or Deceased Eligible Veterans or National Guard Members”.   This can be a copy of a current Washington State Driver’s License, death certificate, or other document showing Washington State domiciliary of the Veteran.  (This document must be provided once each year while the dependent is in school using the GI Bill.)
