Degree Application (PDF File)
Students nearing completion of the requirements for a degree should submit a completed degree application to the Office of the Registrar.

Students seeking high school credits for Peninsula College courses should complete this form.

Incomplete Agreement (PDF File)
A grade of “I”, incomplete must be initiated by the student.

Medical Withdrawal Form (PDF File)
Students encountering medical difficulties requiring withdrawal should have their doctor complete this form.

Petition To Disregard Grades
Students returning to Peninsula College after two or more years may petition to have grades of less than 1.4 disregarded in computing their GPA.

Prior Transcript Request (PDF File)
Enrolling students who have previously attended college at other institutions may use this form to request a transcript.

Repeated Course (PDF File)
A course may be repeated one time. The higher of the two grades received will be used in computing grade point averages. Use this form to notify the Enrollment Services Office of a desired GPA recalculation.

Reciprocity Agreement Request Form (PDF File)
Reciprocity is a new policy among Washington community and technical colleges (CTCs). It is designed to assist students in transferring courses that have met Communication Skills, Quantitative Skills, or Distribution requirements of transfer degrees from one CTC to another. Students can request reciprocity of individual courses and/or reciprocity of an entire distribution area(s).

Request For Change Of Advisor (PDF File)
To change advisors, students should complete this form and return it to Counseling Services one month prior to the beginning of the next quarter.

Transcript Evaluation Request (PDF File)
Students may request a transcript evaluation to determine whether all degree or certificate requirements have been met.

To obtain an official transcript, signed by the registrar , complete this form, pay $2.00 per transcript, and submit the form with the receipt to the Enrollment Services Office.
